Sazetak: U cilju boljeg i bržeg zbrinjavanja kritično obolelih i povređenih postoji tendencija uvođenja i korišćenja bodovnih sistema u proceni težine zdravstvenog stanja i praćenju ishoda lečenja. Procena zdravstvenog stanja pacijenta putem bodovnog sistema prilikom urgentnog prijema može unaprediti tretman i biti novo oruđe u trijaži obolelih i povređenih u prehospitalnim uslovima. Akutna fiziološka i procena hroničnog zdravstvenog stanja, APACHE II skor (Acute Physiology And Chronic Health Evaluation) danas je najšire korišćen skor u jedinicama intenzivne terapije (JIT). Ovaj skor bazira na dvanaest fizioloških i biohemijskih parametara koji se svakodnevno određuju radi procene trenutnog stanja pacijenta. Biohemijske analize, neophodne za ovaj bodovni sistem, čine ga nepogodnim za upotrebu u prehospitalnim uslovima. U težnji da se dobije bodovni sistem koji će imati sposobnost predikcije ishoda, a bez biohemijskih analiza, razvijen je REMS bodovni sistem (Rapid Emergency Medicine Score) koji pruža mogućnost brzog i jednostavnog kvantifikovanja bolesti, u prehospitalnim, i u hospitalnim uslovima. Uvođenje bodovnih sistema u svakodnevnu praksu omogućava analizu i poređenje rezultata terapije i zdravstvene nege.

In order to attend to the critically ill and injured more quickly and effectively there is a tendency of putting in and using scoring systems in the assessment of seriousness of medical state and attending to the outcome of treatment. The assessment of health state using scoring systems during the urgent admission could advance or promote treatment and be a new tool in deciding on the priority among critically ill and hurt people in pre-hospital conditions. APACHE II scoring system (Acute Physiology And Chronic Health Evaluation) today is the most widely used score in Intensive Care Units (ICU). This score is based on twelve physiological and biochemical parameters which are daily scored in the assessment of momentary state of the patient. Biochemical analyses, necessary for this scoring system, makes it unfit for use in pre-hospital conditions. In a desire to get a scoring system with an ability to predict results without using biochemical analyses REMS score was developed. REMS score gives you the possibility to quickly and simply quantify illness, in pre-hospital as well as in hospital conditions. Putting scoring systems in every-day practice enables analysis and compare-son of results of therapy and health care.
